CRITICAL CARE SERVICES
Our Critical Care Program (CCP) offers two pathways for patients receiving specialist care while as an inpatient in hospital, and as an outpatient in the community.
CRITICAL CARE
While the patient is in the hospital, our CCP platform allows our expert staff specialist and nurses to evaluate and manage the treatment of critically ill patients from a centralized command center to regional hospitals within an Area Health District. Patients will continue to be monitored and cared by local regional hospital staff, but the integrated model will allow the following benefits:
- Smoothen the transition for escalation of care management. We aim for the model to reduce the time for intervention of care.
- Reduce financial burden to a regional and rural health service by reducing lengths of stay, improving the outcomes of the patient care delivery
- Increasing the retention of health workers by alleviating the workloads and increasing the job satisfaction.
LONG TERM CARDIOVASCULAR CARE
The aim for health delivery is for the individual to have access to quality care regardless of their location. Our health technology augments the delivery of personalized care for patients with long term conditions at their home settings, assists with improving their quality of life and prevent avoidable hospital admissions. The use of this technology allows the following:
- Provides support for local community organizations, particularly in remote and rural settings
- Patients and their families will have direct access to their specialists without leaving their homes or communities.
