Our COMMUNITY & INTEGRATED CARE Program integrates primary, secondary, community, pharmacy, social, and mental health providers. The aim is to improve the patient experience and offer more proactive care in order to decrease emergency admissions by 17 percent and nursing home admissions by 10 percent for diabetics and the elderly, reducing the cost of care for these groups by 20 percent over the next 12 years.

HIA-Preventative Care and Research is an extension of our commitment to lead research, clinical and educational initiatives that support the tailoring of medical treatment to the individual characteristics and preferences of each patient. HIA Health Conferences are aim to educate, engage and encourage our communities to improve their health. To assess healthcare challenges and perform gap analysis and then designed program improvements based on needs, resources, and prioritization.
